Needledrop Licenses: Please see your price key to determine amount of license fee. We will bill you according to the type of usage that you report on this form.

Blanket Licenses: No additional synchronization fees will be charged as a result of using this form, however you are still required to report all usage.

   PRODUCTION FOR SALE
Production sold through catalogs or Corporate, Educational or Government auspices and not available to the general public or any production which is sold through mass marketing channels and is available for purchase by the general public.
   AUDIO/VISUAL PRODUCTION
Any production such as "Corporate, Educational, Governmental, etc.", which is not for broadcast, sale or public showing where a fee is charged.
   MULTIMEDIA PRODUCTION
Interactive (non-linear) productions, such as CD-ROMs, CDI, Floppy Disk or Hard Disk.
   THEATRICAL PRODUCTION
Trailers, Commercials, Films, Festivals, and Theme Parks.
